Filling out this form sets up a data file for your school. To preserve the integrity of your data, every user must sign in with the exact same school name, city and state. There will probably be fewer errors if you tell students to use just the "main" name, like Kennedy or Lincoln, without adding Middle School, High School etc.
If you intend to have the students complete the survey more than once to compare pre- and post- an intervention, you must add the suffix -Pre or -Post (like Kennedy-Post to the school name. If you're not sure whether you'll want more data later, still use the -pre suffix to keep your options open.
To control the integrity of the data, while protecting user anonymity, and to separate pre-intervention data from post intervention data, we require that you limit the amount of time the survey is open. Your open time starts as soon as you press the register button - the close date is the one you enter as a date below. To be able to input data, you MUST enter a stop date after today.
Remember to separately register a School-Pre and a School-Post (as if they were separate schools), and give them separate open periods to get the data you want for comparison.
You need to select a password to register. You share that password with teachers, but not students. This password is what separates student data from teacher data, so it's important for all teachers to use it. Students don't need a password.
When you enter an email address, the system will email your sign-in information, and instructions for use, directly to you. You can then use this to notify staff about the profiler. We don't use your email in any other way, and we will never share it. For details, see our privacy statement.
